Montana readiness assessments for hemophilia specialty access focus on documented gaps in on-site hematology coverage across its 27 rural counties. Funded initiatives must produce verified telehealth linkage rates and measurable reductions in one-way patient travel exceeding 150 miles.
Montana's frontier county designation means 47 percent of residents live more than 100 miles from the nearest hemophilia treatment center. Workforce data show fewer than 0.8 hematologists per 100,000 residents, concentrated almost entirely in Billings and Missoula. Any readiness proposal must therefore quantify existing broadband upload speeds at proposed spoke sites and list exact satellite or fiber redundancy plans.
Implementation requires formal memoranda with the Montana Telehealth Network and the state's two level-II trauma centers. These agreements must specify shared electronic health record access protocols and on-call hematology coverage windows. Applications that omit upload-speed verification or trauma-center sign-off are returned without review.
Montana's approach differs from Idaho's because it requires explicit demonstration of extreme rural delivery capacity due to frontier county designation and documented winter road closure statistics.
