Montana's workforce constraints limit the number of clinicians available to deliver personalized chronic care via telehealth across 56 counties. The state records 1.1 physicians per 1,000 residents, with 42 counties classified as health professional shortage areas for primary care. Funding must address the shortage of personnel who can interpret individualized data streams from remote monitoring devices used by patients with diabetes, heart failure, and COPD.

These constraints concentrate in the eastern and central regions where distances between county seats exceed 100 miles and winter road closures are routine. The absence of sufficient trained staff means that data generated by home sensors often remain unanalyzed, reducing the value of individualized treatment algorithms. Projects must therefore expand the pool of community health workers and registered nurses who can review patient-specific dashboards and escalate anomalies to distant specialists.

Infrastructure constraints compound the workforce gap. Montana's broadband subscription rate stands at 72 percent statewide but falls to 48 percent on reservations and in counties east of the Continental Divide. Existing rural health clinics operate with limited IT staff, restricting their ability to integrate new data feeds into electronic health records. Applications must document how additional personnel will maintain data quality and privacy standards under these conditions.

Readiness requirements include demonstrated capacity to train at least 25 new telehealth facilitators per project year using curricula that emphasize interpretation of continuous glucose or blood pressure trends for individual patients. Sites must also show existing agreements with at least two tribal health organizations or critical access hospitals to host training cohorts.

Unlike Idaho's focus on cross-state specialist networks, Montana applications require proof that expanded workforce capacity will remain within the state's 147,000 square miles and will not rely on out-of-state contractors. Quarterly reporting must include workforce retention rates at six and twelve months post-training to confirm that gains in data-driven chronic care delivery are sustained.

Projects that cannot provide county-level maps showing current versus projected facilitator coverage will not advance.
