These outcomes matter because Montana's population density stands at 7.1 persons per square mile, the second-lowest in the nation. Agricultural and energy sectors employ 14 percent of the workforce, leaving few local clinicians available for ongoing senior care. Counties along the Hi-Line report diabetes-related hospitalization rates 2.8 times the national average for adults over 70.
Implementation requires documented partnerships with one of Montana's 51 critical-access hospitals and the Indian Health Service units serving the seven reservations. Awards specify that grantees must submit quarterly claims data from Medicare and Medicaid showing visit completion rates above 65 percent for enrolled participants. Broadband mapping from the Montana Department of Administration identifies 14 counties where fixed wireless or satellite remains the only option; proposals must budget for equipment compatible with those connections.
Unlike Idaho or Wyoming applications, Montana requires demonstration of extreme rural delivery capacity due to frontier county designation. Grantees must also align reporting with the state's existing telehealth reimbursement rules under Medicaid, which cap originating-site fees at $20 per session.
