Montana's frontier counties require documented readiness for data collection and quality reporting before funds are released. Applicants must already operate an electronic system that captures encounter-level data on immunization status, blood pressure, and body-mass index with less than 5 percent missing fields. This threshold filters out organizations still using paper logs common in counties with populations under 8,000.
Facilities located on the Hi-Line or in the southwestern mining districts must show that broadband connections meet 25 Mbps download minimums during peak hours, verified by three speed tests submitted with the application. Counties without fiber backhaul, such as Garfield and Petroleum, need letters from providers confirming planned upgrades within 12 months or alternative satellite arrangements with latency under 600 ms. Workforce constraints compound these infrastructure requirements: the state reports 14.8 primary-care physicians per 100,000 residents in frontier areas versus 82.3 in Missoula County.
Readiness also encompasses partnership agreements with local boards of health and tribal health departments. Proposals must include signed memoranda confirming that each partner will share de-identified aggregate outcome data quarterly. Projects lacking these agreements at submission are returned without review.
Montana's county-based public health structure differs from the district model used in Idaho and Wyoming; therefore applications must map reporting lines to individual county commissioners rather than regional health officers. Reviewers further require a 12-month cash-flow projection showing that equipment maintenance reserves equal at least 8 percent of requested equipment line items, reflecting the higher freight costs of shipping parts to ZIP codes east of the Continental Divide.
